NEW PATIENTS

Process for making an appointment:

1. Fill out the contact form below

2. Be prepared to submit the following documents:

   • A copy of your driver’s license and insurance card

   • $100 appointment deposit*

3. A Bridgepoint representative will reach out to you within 48 hours to schedule an appointment.

*All new patient appointments will require a $100 appointment deposit that will be applied to your copay, deductible, or coinsurance at your first visit. To avoid a late cancellation fee, please make sure to cancel your appointment at least 24 hours in advance, on the business day preceding your scheduled appointment. Per our office policy, we charge a $100 no-show/late cancellation fee.

Patients with no insurance can self-pay at the time of the appointment but will still be required to pay the new patient deposit.

Note: We do not require a professional referral to be a patient at Bridgepoint Clinic.
